Person Centred Planning

It is a natural phenomenon that every individual thinks and plans his or her life in different ways. Fortunately, some people manage to have a clear vision about their goals and objectives and many others live on the basis of 'Come what May'. These people do not plan their individual goals and means to achieve them; rather they prefer opportunities as they arise. The primary objective is to fulfil our dreams and match them with reality. People who look up for assistance with this task go for Person Centred Planning (PCP). This planning process is the structured way to make a person's dreams come true and enabling people with mental or physical disabilities to successfully achieve social inclusion.

The person centred planning does not only cover the person, but seeks to share the power of the individual with family, friends and the community as a whole. The family persons are the ones who better know the focus person and can highly contribute to make adequate changes in improving his or her life. Active involvement from each key area is an important feature of the entire plan.

6 Stages of PCP

The process of person centred planning comprise of 6 stages that are as follows:

Stage 1: Gather person centred information

The first stage is of gathering person centred information through support plans, person centred plans and person centred reviews. These three sources primarily satisfy the questions like what is working and what is not working for the focus person, and consequently developing plans for future improvement.

Stage 2: Transfer the information into useable format

The second step is to form the information in such a way that it is presentable and useable during all the workshops. This process depends on the number of people who have participated in providing information. The recording is usually done before the meeting if the reviews are more than 30 or at the meeting if they are less than 30.
