Introduction

There is a need to conduct a risk analysis with the help of using causative factors. These factors will be considered by meeting the issues related to the event described in the upcoming paragraph. The rating of the risks will be carried and will have a total of nine causes. There is a scenario in which a 67 year old man, known as Mr. B entered an Emergency Department of the hospital after blacking out and dislocating the shoulder. At the moment, there are nine issues which are arranged in a particular sequence. In this case, a hospital protocol was not followed for IV sedation in the ED like continuous monitoring via B/P, ECG and pulse-ox until the patient was able to remain conscious again. There were higher than normal doses of sedation which were administered by nurse on the order of Dr. T. The history of the patient concerned with lower BP was analyzed and it was seen that he has not undergone regular tests over the period of time. The LNP did not pay attention to the warning signs of the Pulse-Ox that had Mr. B in peril. It showed that Nurse J was not communicating with the LPN in regard to her pt. Dr. T made a big mistake of not checking the pt post anesthesia of the patient. The backup staff was not utilized in a proper manner as the Emergency Department became very busy with their work. In principle, the staff should have been called for assistance. The history of the patient in regards to the gastric reflux and obesity must have been assessed in detail in regards to the position concerned with sedation. There was not a single mention of advanced directive question concerned with the arrival of Mr. B who was placed on a ventilator against family wishes. The families were made to make a tough decision when they were informed about the patient of having a brain death in the end. Therefore, all the issues related to risk management regarding this issue will be discussed in detail.

Discussion

It was a total case of carelessness from the doctor's side which led to such a situation in the end. The emergency department did not perform their work in a proper manner which led to such a situation. Normally, in hospitals, it is seen that the Emergency Department have a high percentage rate and it is the only area where the patients are given maximum attention under critical situations. However, the condition of the patients matters considerably in such situations. When the condition of the patient is not critical, they are discharged after few hours after going through a detailed examination. In few cases, there are patients who try to visit an Emergency Department in order to have quick attention and care from the concerned doctors.
