Introduction

The expression care management or Medical homes are interlinked. One of the key factors of care management is that client care is managed and coordinated. Increasing access to a medical home is one of the best strategies used in developing programs of care management. (Browdie, 1992)

Care Management

Programs of care management systems applies science, systems, information and incentives to get better medical practice and helps in consumer assistance and their support system to become engaged in a collaborative process designed for the management of mental/social/health and medical conditions more efficiently and effectively. Care management's goal is to achieve an optimal level of improve coordination of care while providing good and services which are not duplicative but cost effective. (Palgrave, 2000)

Care Management Components:-

Prioritization and Identification:

Identify clients at the highest risk who those clients who offer improvements at greatest potential are identified at the highest risk in health care outcomes.

Strategies Tools/: predictive modeling, referrals, Health assessments surveys, referrals.

Intervention:

Intervention must improve quality and cost effectiveness. It must be customized to meet individual client's needs and involve the client in planning and decision making.

Strategies/Tools: patient self management education, care planning, evidence based practice guidelines, collaborative practice models, integrates physical and behavioral health.

Evaluation

Includes testing and analysis to be sure interventions, systematic measurement, improve quality, effectiveness and efficiency.

Strategies/Tools: utilize known measures, Program evaluation

Payment/Financing

Compensation should be re-aligned and maintained reward providers for quality.

Strategies/Tools: expanded payments for care management, Pay for Performance, and Medical Home related services, e.g. expanded clinic hours or care planning activities.

Managerial Challenges

There are six main challenges faced by the managers, environment filled with pressure social demands, public policy and professional mandates in the field of public health while leading an organization. Foundation is been formed by these challenges to work and need to be performed. By the accomplishments of challenges successes' variety can impact the organization when delivering the effective health outcomes, for creating effective performance of the organization, and contributions towards broader public health profession. Immediate and tangible benefits are being offered for employees and staff, and for the external clients, the general community and patients. Every challenge gives chance for managers of health care for developing strategies and approaches that convert the challenges to organizational benefits especially. (Routledge, 1999)

Care Management Targets

Care management's target is to build a healthcare application covering all the requirements of the hospitals in a country and allocating them to correspond clinical data globally. An open source of technology is to be used. Application includes Health Level that provides standards for the exchange of data among worldwide healthcare computing applications. Several other standards are also used. (Doherty, 2002)

Every major health plan has Care management. There should be programs to reduce the medical cost. With the administration focusing on health care reform and a continuing economic crisis, the need for more effective and less costly models of health care has never been more pressing.
