The health problem chosen is chronic kidney disease. Chronic kidney disease is a progressive loss (for 3 months or more) and irreversible kidney function. The degree of involvement is determined by a glomerular filtration rate (GFR) < 60 mL/min/1.73 m 2. As a result, the kidneys lose their ability to remove wastes, concentrate urine and conserve electrolytes in the blood. Symptoms of deterioration of renal function are nonspecific and may include a feeling of general discomfort and reduced appetite. This would help prevention of the disease and lead towards the success of the program when the experts would join hands together and bring out prevention steps and guidelines (Imran, 2011).

Evidence for program intervention

This proposal aims to create a knowledge base and to outline organizational the prevention of kidney function (primary prevention), to slow down their evolution (Secondary prevention) and to prevent the devastating cardiovascular complications triggered renal insufficiency (tertiary prevention).
