Introduction

Mr. Tosack has been in hospital for the past 2 weeks and finally he being discharged after his hip replacement treatment. He has to use a walker for the purpose of moving and walking due to treatment. Mr. Tosack lives alone; his wife died 2 years later. He has one son who does not live with him but near to his bakery. The son has been working hard throughout the week and does not get time to think over other matters. Mr. Trosack's injury is not a very serious one if adequate care gets taken after the treatment.  Mr. Trosack lives alone in his apartment which gets located on the 2nd floor of the building and does not have any elevator (Cockcroft, 1976).Discussion

Misunderstandings about discharge options may lead to delayed discharges and unnecessary stress on patient and family caregiver, as well as on staff. Diabetes of Mr.Trosack

The biggest healthcare issue in the discharge of Mr. Trosack is that he is a diabetic patient. His children were not aware of this health  issue faced by Mr. Trosack.  According to them, their father has always been physically fit, and there were no health problems associated with him. Health informatics catches on. Hygiene Issue

As Mr.Trosack lives alone in his apartment which locate on the 2nd floor of the building, he has the problem and issue of going downstairs. This is because after the discharge, he has to be assisted by a walker. The garbage of the house has to be dumped into the ground floor of the building which is not possible if Mr.Trosack is living alone. Apart from this, the refrigerator at Mr. Trosack's home contains only expired food which is very harmful to him.  Mr. Trosack usually eats at his bakery which means he is often consuming outdoor food that results in more hygienic issue.

Hypertension

Mr. Trosack has been a patient of hyper tension and prescribe to the use of Lopressor 25mg. He is also a diabetic patient as he is overweight.Interdisciplinary Team Members

Mr.Trosack gets fully involved in planning for their discharge and keep will be kept informed of any change affecting agreed plans.
