6.1 Critically analyse and demonatrate the development of effective relationships with learners with in the context of practice

The relationship between learners, clinicians and faculty therefore must shift to a partnership based on mutual learning and understanding rather than the hierarchical and adversarial teacher-student relationship. An innovative conceptual framework with a healthy dissonance toward existing situations is needed to prepare educators and clinicians for the 21st century. Skill competency is an essential component of undergraduate nursing education and a societal expectation of nursing graduates. However, an undergraduate programme that focuses on clinical proficiency while minimizing emancipatory thinking is not preparing a professional for the 21st century.

The nursing care in the Dialysis Unit is a mixture of Team and Primary Nursing. Hegyvary (1982) cites that primary nursing is both a philosophy of care and an organisational design and it is not simply a way of assigning nurses to the patients, but rather a view of nursing as a professional, patient centred practice. Skidmore (2001) defines primary nursing as a system for the distribution of nursing care in which care of one patient is managed by one nurse in the entire shift, and co-ordinates with other nurses and medical personnel, schedules all tests, procedures and daily activities for that patient. In an acute care situation learner found the Haemodialysis Unit, one nurse is responsible for one patient; the advantages are that the continuity of care, accountability, and the patient centred care is individualised and co-ordinated(Uer , 2007, 73). The Dialysis Unit is aiming to have one "Acute Nurse" every shift in order to take responsibility for any acute patients needing to be dialysed in the unit or in the ward (responsibilities of acute/on-call nurse). Students' evaluation of this simulation activity showed they considered it to be successful. Students liked the fact that practicing midwives had validated the items and decision rules indicating they felt this provided clinical credibility gives another definition of primary nursing, citing that "the nursing care of a specific patient is under the continuous guidance of one nurse from admission to discharge, round the clock care is coordinated for each patient by the nurse designated as the primary nurse for that patient. Named nurses in The Haemodialysis Unit are all General Nurses, registered by the Nursing Midwifery Council. These RGN's are responsible for assessing, planning, implementing and evaluating the patient's care. It caries important legal and professional implications as accountability and responsibility are increased. The Named Nurse must always liase with the Multidisciplinary Team to plan appropriate and individualised care(Beer , 2005, 591).
