Discussion

“…Health care is increasingly a team effort. No one health professional is able to offer a

patient (or client or service user), the full range of services” (Thistlethwaite, 2008). The health team in Ireland is made up of a wide range of health care regulated, unregulated, informal caregivers and volunteers. The greatest groups of unregulated care providers are members of the family, friends and community volunteers.

Lifelong learning is an important part of working life of many health care professionals. It is not easy to ensure that we have the right number of providers care with the right mix of skills and training to provide care the right place at the right time. That depends on several factors, including trends demographic, health, technology, practice patterns, and the organization and delivery of health services. The skills and roles of health professionals are not the same across the country and often overlap. They also change over time. Even within the same profession, the roles and skills may vary. Thus, the practice of family medicine in urban areas is very different from the exercise area rural and remote areas. As part of primary care reform, the focus is more that physicians, nurse practitioners and other providers work in teams to provide services.

Nurses have an important role coaching, facilitation and support from caregivers. Their scope is very broad; geriatrics is a place where they can fully express their own role. Among the group care, there are support personnel, consisting of a nurse and a supervisor head. Their role is very important to clarify objectives, organize, motivate and develop teams. The initiative builds on previous work and research that confirms the benefits of interdisciplinary collaboration for patients / clients, communities and care professionals primary health care.
