CHAPTER 01- INTRODUCTION

Outline of the Study

The topic of this study is “what is the perception of quality of life older persons living in a adapted micro-environment?” The first chapter of this study will introduce the topic and the background of the research along with the purpose and the significance of the study. The second chapter of this research will provide the literature review that will present the related research with highlighted facts and figures of this field. The third chapter covers the methodology for this research. In the methodology the philosophy of the research will be defined along with the data collection method and the tools that will be used for this study.

Background of the Study

The intermediate healthcare is intended to provide the facilitation for the transition from hospital to home while the care objectives are non medical like, hospitals discharge the patient earlier or the length of stay at hospital is decreased. Corresponding to the fundamental of care which is close to home, the intermediate care should be given in settings based on community in general, such as, in the home of the patient or in discrete step down facilities on the site of the hospital (Charles & Sevak, 2005, pp.1174-1190).

Since the number of elder people, who need hospital care, is increasing and the pressure on bed along with the identification of increasing issues related to discharge from the hospital are increasing, the requirement for the effective discharge planning for old age people has become highly significant. The core issues are the lack of evaluation and plan of discharge with the patient and his caregiver is very rare, poor communication between communities and the hospital, extra dependability on informal support or poor provision of legal service. Moreover, the patients are given little attention (Buhler, 2007, pp.253-259).

Discharge planning is intended to analyse the present healthcare facility in compliance with prescribed treatment, improvement in the home care and safety and prevention of unimportant admission of patients in hospitals and promotion of efficient treatment service. It is intended to make hospitals able to discharge the patient early and prevent the not required prolonged hospital admissions. In UK, this type of discharge is practiced in local hospitals. In each local hospital an operative care centre has an objective to manage the community hospital, residential and homecare services for the patients who are discharged from the hospitals. The problems of patient who are discharged from the hospitals can be different or can change with time. These issues can be regarding their, social, functional, emotional or physical status and they may include factors related to the patient, care or the features regarding the social circle of the patient. The problems of the patient are may be regarding the decreased health or functional status or some emotional disturbance.
