Introduction

Individualized nursing plan provided to an individual is refer to as teaching plan in nursing. It is a set of behaviour that a nurse has to apply in order to help support or resolve nursing diagnoses. It guides and assists advancing nursing care and evaluates that care. A chronic need is refer to as diabetes, which is incurable. For a permanent change, this disease requires lifetime commitment, facilitate those diagnosed to control and mange their disease. To fulfil this education must be given with medical treatment, hence needing a diabetic teaching plan. Most people diagnosed with diabetes lack knowledge about these diseases and may require education. The aim and objective of a teaching plan is to instruct the individuals suffering from diabetes (Long.B.C, 1965).

Teaching them about the importance of selection they will have to make and how it affects their disease. Because two patients are not the same, and a different plan is being formulate and modified to their health needs, permitting them to have the highest eminence of life possible. In this teaching plan, we are interested in nursing care for two newly diagnosed patients namely a 10 year old and a 35 year old suffering from Type 1 diabetes.

Clients background

A 10 year old school girl Sara Reynaldo suffering from Diabetes type 1 recently was refer for a teaching plan. Her parents were very concerned about their daughter's situation. She was very attentive and cooperative experiencing spells of nightly hypoglycemia, her bleakness of improvement led her to follow a diabetic teaching plan.

Another 35 years old male Jacob Francis diagnose with diabetes type 1 who had no family history, no prior symptoms was only about 10 pounds overweight and fairly active.

Assessment of Developmental or Cultural & Specific Client Learning Needs

Diabetes patient has to understand diabetes mellitus or type 1diabetes as widespread with other chronic illness. Diagnosis is bound to be devastating and unexpected. The most common response for patients is grief, and a resolution of grief is mostly dependent on factors such as education, geography, economics and religious/cultural factors. Family and friends support mainly affects long-term acceptance. Patients need to consult a professional if they are suffering from depression which is very common with all other chronic diseases like diabetes. Depression should, therefore, be treated immediately to control depression and thus be able to manage the diabetes.
