Disease Management

In this paper offers a reflection on what is a model of care and how it can be done processing. There are several concepts about what it means to model and ways of structuring knowledge of nursing. The model of care concepts and ways of developing knowledge structures presented here are philosophers and nurses. The concept of care is constructed from empirical scientific knowledge and professional, culture and beliefs, both professional and of being that is careful and his companion, as well as the institution and the environment in which they located.

These concepts are complementary in certain times, diverge in others, or we help in the search for new concepts. Nursing knowledge is classified and structured according to a greater or lesser degree of abstraction. The structure of knowledge in decreasing order of abstraction can be defined as meta-paradigm, philosophy, conceptual models, theories and empirical indicators. All elements of the structure of knowledge, except for empirical indicators are composed of concepts and propositions.

The meta-paradigm is more global and more abstract discipline. In its structure there are four different concepts conceptual meta-paradigmal: human, environment, health and nursing. Philosophy is the second element of this structure, which can be defined as an ontological statement about the central phenomenon of interest in the discipline, an epistemological statement about how this phenomenon is seen, and an ethical claim about what members the discipline assessed. The third element is the conceptual model, which is defined as a set of related abstractions and concepts that guide the central phenomenon of interest in the discipline. There are propositions that are generally described these concepts, as well as the propositions that assert the relative abstraction and general relations between two or more concepts. The conceptual model stands for several terms, such as conceptual system, "framework" paradigm and disciplinary matrix. [We must clarify that there is no global consensus on the concept of paradigm. The use of the meaning of paradigm is wide, varied and undefined, but there is some consensus when it is conceptualized as a prime example of a category of objects. The same author complements his discussion by defining it as a model or a way of knowing that includes an understanding of individual and an understanding of the world he lives and relations ...
