Introduction

The aim of the paper is to extricate the medication errors that arise due to various reasons such as wrong prescription, wrong timings, wrong dosage, misleading packaging and so on. The medication based errors are categorized as knowledge based errors, skill based errors, memory based errors and activity based error. The paper extricates the reasons behind the errors as well as appropriate prevention to avoid these errors. Moreover, the paper highlights the government policies for the mitigation medication errors. The paper has intention to highlight the importance of training and development in the health organizations in order to avoid medication error. The paper also discusses the incorporation of information technology based medication processes to mitigate the probability of errors.

Medical error

Medical error is basically an issue of negative or inappropriate impacts of medicines on the health of the patient due the medication process of the diseases. This error usually occurs due to wrong meditate or it may arise by providing inappropriate prescription regarding the dosage of the medicines. In addition to it, there are so many other reasons that may cause an error in the medication process such as, an error may arise if the staff of the hospitals and the nursing homes in not enough to cater all the patients efficiently or if the staff of the hospital is inexperienced. Moreover, if the staffs of the hospital and the nursing home are lethargic and reluctant to follow the pre defined procedure of medication of the hospital then their laziness may damage the efficiency of the medication process (Montesi G. & Lechi A., 2009).

There could be different reasons behind the error in the medication process such as due to the wrong dosage or due to providing meditation service to the wrong patient. In addition to it, the medication process becomes faulty if the timing o the dosages are not appropriate and if the staff is prescribing wrong medicines to the patient. This extricates the nature and importance of the crucial relationship in between the health of the patient and the responsibility of the staff of the health care service providers (Lisby, M., et al., 2010).

In addition to these causes, an error in the medication process can arise due to the wrong manufacturing and formulation. For instances, an error may occur with misleading packing as well as due to the containments or adulterants. In the same way, a medication error occurs with an inappropriate use odd rungs and biological such as wrong dose as well as wrong rout and inappropriate quantity. Timing is another sensitive aspect of the medication error. Wrong timings of biological and drug usage can damage the effectiveness of the medication and may exert a negative impact on the health of the patient.
