Task 1

Activity 1

At the time of admitting a patient, measuring the vital signs of the patient is one of the most important responsibilities of a duty nurse. This measurement of vital signs in early hours not only save the time of a doctor, but also is substantial to diagnose the patient properly.

Equipments Used in Measuring Vital Signs

The initial equipments that an on duty nurse uses in order to measure the vital signs of the patient include the following;

A Thermometer to measure body temperature

A Stethoscope to measure pulse and to observe respiration

A Sphygmomanometer along with a stethoscope to take the blood pressure of patient

Verification of the Data

Verification of data stands for the procedure to carefully examine the obtained information to make sure the maximum levels of quality care. It can be performed by double checking of findings, within the similar situations after a short period. It will make sure that there are no variations in measurements and is absolutely appropriate (Funnell et al, 2008, pp. 34).

The Importance/ Impact of Collecting Data on Admission And/ Or Discharge

The admission and the discharge of a patient denote the commencement and the ending of hospital stay of a patient. These are equally the most critical phases in order to provide best health care to patients. On admission, appropriate data should be obtained, which include indemnity information, blood relation, medical history, allergic reactions, previous treatment (if any), swap outlook and it is imperative to identify specific aspects that make sure the continuous progress of the patient's recovery at the time of discharge.

Holistic Care of Client and Family

Nurse must provide holistic care to the patients and their families by ensuring they feel comfort, and become familiar with the functions and responsibilities of medical staff. Ample information of therapeutic procedures should be provided to them and proper consent must be obtained. It must be kept in mind what they believe. In order to provide holistic care occupational therapist is supposed to visit their house and analyze the domestic situation and must recommend any variation if needed (Hoban, 2003, pp. 54).

Documentation, History Collection and Following of Policy

Gathering appropriate history from patients leads to better decision making in order to provide apposite care to them and to ensure their recovery at a healthy pace (Hoban, 2003, pp. 54). The participation of patients in treatment procedures facilitates nurse and doctors to identify therapeutic needs and processes of fulfillment so that needs could be satisfactorily met. However, an expected date of discharge can be assumed and must be noted in patient's folder at the time of admission. Some delay is probable in the discharge date.

Letter to GP

When the patient is going to be discharged, a letter must be issued to the GP at the time of discharge, which contains particulars of hospital stay of patients including signs/symptoms of disease, treatment methods and patient's response towards treatment, success of cure and further progress of patient (Mahadavan, et al. 2009, pp. 397).
