CHAPTER 01: INTRODUCTON

Background of the Research

In the United Kingdom, there are around 1800 cases of breech delivery in a year. The majority of breech delivery cases were handled by obstetrician. Midwives only handle a few of the cases of breech delivery. Realistically, an average midwife, in the United Kingdom, handles only one or less than one case of breech delivery.

Breech presentation takes place in around 3 to 4% of term pregnancies. The chances of neonatal morbidity, Perinatal mortality, and other problems are higher as compared to pregnancies with cephalic position of the foetus (Ho, 1986, pp. 1184).

In cases of breech births, it is important to adopt the right approach. There are two options that could be selected in breech presentations including vaginal birth or a caesarean. Midwives usually go for the vaginal birth even in the case of breech presentation. However, a majority of obstetricians prefer a caesarean. There are different studies which have been conducted to identify the recommended method of delivery in cases of breech presentations. The delivery of foetus in breech presentations has long been a subject of controversy. There are diverse studies which have conducted on the outcomes of vaginal delivery or a caesarean (Hassall, Dunlop and Lewis, 1996, pp. 28).

Problem Statement

According to the studies conducted on the topic of breech delivery, midwives are losing their skills to conduct breech deliveries normally. According to health experts, a number of women are having caesareans which show that midwives are unable to deliver breech babies normally. The increased acceptance of caesarean could also be because of the reason that child bearing women and their families have now become more conscious about delivery procedures and care.

Midwives have long been dealing with cases of breech delivers. However, in the current scenario, the situation is different. Currently, health experts and obstetricians prefer a caesarean instead of vaginal birth in breech presentations. It is because they believe that vaginal birth at home without medical treatment is risky for both mother and the baby. Therefore, midwives do not get enough breech delivery cases to deal with. In order to solve the problems and complications associated with breech deliveries, it is important to identify whether midwives should conduct breech deliveries or not.

Purpose of the Research

The purpose of this research is to determine whether midwives should conduct breach deliveries or not. The topic has been selected for this research because breech delivery is an important issue in nursing and midwife education. The issue is equally important for obstetricians and gynaecologists.
