Circumventing Medication Error Using HER

Introduction

The conventional workflow in the caring processes is usually centered on the structure or required clinic paperwork. The impacts of an alternative workflow would be caused by a transition from a paper-based clinical workflow to an electronic-based one. These impacts may not only cause uncertainties about the adoption of EHRs in the healthcare settings, but also alter or disturb routine workflow, such as increasing the administrative workload of care providers and prolonging the caring time (O'Connor et.al, 2011). The purposes of using EHRs are to provide cares in quality, safety, and efficiency in the healthcare settings. Therefore, reducing documentation time or simplifying the workflow of care should be an expected benefit of using EHRs (Weber et.al, 2008).

Workflow Issue

The workflow issue that has been identified in this study is associated with the increasing number of medication management and the prescription errors among Diabetes patients (Pate et.al, 2002). According to the statistics 21 % of the patients suffering from Diabetes have faced prescription error due to the wrong information provided to the doctor concerning the patient's condition. Along, with that more than 20 % of the patients have medication management errors, which occurred due to the wrong patient record files the doctors failed to retrieve patient's history and adjust their medication when the patient has a high blood glucose level (Sequist et.al, 2005).

Relevance of the Workflow Issues with EHR

The error of prescription and effective medication management is related to the EHR. Since the organization does not have an efficient EHR system in place they keep paper records of the patient's medication management and prescriptions, which is not an efficient manner to record patient's history (Crosson et.al, 2007). There have been cases where the nurse have failed to update the medication provided to the patient, and in some cases these paper records have been switched from one patient to another causing perilous situation inside the hospital.

Objectives Realted to the WorkFlow

The objective of this study is to provide the health practitioners with an effective and efficient HER system. A system which would enable them to keep patient medication records, the system will possess clinical summaries so that the patients suffering from diabetes would easily be able to engage in and coordinate with their care service providers. The system will provide the clinicians with essential information required to make decisions related to change in medications. The system will enable the clinicians to efficiently and effectively manage the prescriptions for patients with diabetes

WorkFlow

If a nurse is ordering a medication or procedure for a patient, they will link that medication or procedure to an encounter diagnosis. You have to correlate the test with the diagnosis so that you move that from your

problem list into your encounter list and its addressed then and your orders are entered to that diagnosis.

As long as diabetes was associated with that medication, it would automatically come over.
