Introduction

The aim of this assignment is to discuss the importance of working inter-professionally within a multidisciplinary team to achieve the best patient-centered care. It will demonstrate developments achieved, and the knowledge gained around this area throughout my training and practice to date. The assignment will focus on the inter-professional care for adults within a hospital environment, as 'Advanced Diploma in Adult Nursing' is my course area.

Case study

Personality is an ingrained, enduring pattern of behavior and relating to self, others and the environment: Personality includes perceptions, attitudes, and emotions and this is influenced by biological and genetic make-up, personal development, and interaction with the environment and with other people. However if the traits become inflexible and maladaptive, this will interfere with how the person will respond to emotional distress and function in the society is referred to as personality disorder. My patient is diagnosed with Borderline Personality Disorder (BPD). She is an 18 years old female. She was admitted after a stranger cut her down from a backyard tree at her home that she is currently sharing with her mother. Her boyfriend successfully committed suicide a year ago. She wanted to join him on his death anniversary. She cries for long periods as she misses her boyfriend. Abby (A), like other BPD patients blow events out of proportion. Her relationship with her deceased boyfriend was unstable relationship. This has intensified her use of cocaine and marijuana. She has sense a poor sense of identity and has experienced a great deal of emptiness especially since her boyfriend died. She has cut herself to relief psychic pain and stress as evidenced by scars all over her arms. She has been in the hospital for two weeks now, but has not improved. She doesn't have any off ward privileges as she is high risk for suicide.

During rounds, the psychiatrist, and the social worker suggested that the patient could go for some three hours; she would be accompanied by her mother to visit the boyfriend's graveside for the one year anniversary as she requested this. The Doctor and the social worker thought it was going to decrease her anxiety. The nursing team was alarmed and expressed that Ale has not improved since her admission. The nursing team continued to argue that A did not have any privileges, on or off grounds, for them to know how she was coping outside of the hospital. The nursing team strongly disagreed with the psychiatrist and advocated for the patient to stay in hospital. The psychiatrist said, “I know what you guys are getting into, but I will take the chance and I am almost sure something might happen”. She was ready to take the risk and the nursing team was over ruled, they gave in.
