Introduction

The Dodge City Hospital Foundation Trust (DCHFT) is working together with communities to achieve a vision of healthy groups working simultaneously and Ontarians premier healthy and active inhabits.

This approach to construction healthy groups will:

Improve wellbeing and well-being, reduce dangers to good wellbeing and save wellbeing care costs

Promote partnerships between multiple parts and systems working to promote health

Align provincial, local and localized efforts and leverage joint investments

Integrate and transform current DCHFT programs to support the Ministry's goals and new directions

Make it easier for communities to access services from DCHFT.

The wholesome Communities Fund (HCF) boosts associations to work together to address multiple risk components to good health, in support of the Ministry's centre wellbeing advancement priorities. These include physical activity, sport and recreation healthy eating mental health promotion reducing tobacco use and exposure preventing alcohol/substance misuse preventing injuries.

Purpose

The wholesome Communities Fund (HCF) is a one-window approach to carrying community partnerships to design and consign integrated programs that advance the wellbeing of Ontarians. This new approach builds on the achievement of present Ministry funding programs and will provide more possibilities for associations to request for funding by expanding the scope of eligible health promotion plans to include: personal undertaking, games and recreation healthy consuming tobacco use and exposure wound avoidance substance & alcohol misuse mental wellbeing promotion. The Fund will need increased partnerships amidst localized, local, and provincial associations, producing in more comprehensive wellbeing advancement programs that provide Ontarians with better access to main concern wellbeing advancement programs and services.

The HCF will support provincial associations that are taking a holistic and integrated approach to improving wellbeing and wellness in Ontario. By employed in partnership, associations will have an opening to gain new know-how and to come to a broader audience while supplying more comprehensive programming to Ontarians in their own communities.

The Ministry understands that localized, local and provincial associations are well positioned to identify what programs and approaches will be best suited to their communities. The HCF will help supply the devices and assets that will endow groups to plan plans that competently address local needs. Acommunity is not easily a defined geographic locality, but also includes persons with alike concerns, convictions and values. Organizations/groups applying to the HCF Grants task Stream are encouraged to characterise their own communities.

The Grants Project Stream is a cost-sharing program conceived to help suitable associations conceive possibilities and mobilize groups to become healthier. The program presents cost-sharing support for non-capital tasks that will assist to the wellbeing and wellness of communities.
