Background Information

Jake is a 15 year old boy multiply and intellectually handicapped high school student. Jake is also speech impaired but his responsive vocabulary abilities are sensible and excellent. Jake has a medical analysis of spastic cerebral palsy his hands and legs are very stiff and tight but he is able to use his fingers to switch on a speech interaction system. Jake's excellent responsive abilities make him an applicant for an IEP individualized educational program. Jake is a very helpful boy and he is able to culturally socialize with personnel, he does not socialize with his colleagues as easily because of interaction restrictions. Jake life in a housing structure in the town that is not in a secure community and he is frequently absent a lot of days from school because the lift in his apartment building regularly does not work. Jake gets a Social Security payment every month for around $1000.00 his mom uses the payment for Jake to buy him decent clothes; he rests in a Government funded medical bed. Although his apartment building is not in good shape his apartment and room is set up very nicely. Jake's mom is getting old and her back is disturbing her and for this reason she has thought of putting Jake in a residential space where he would be able to get 24/7 care.

Treatment and Academic Plan

Jake is a Cerebral Palsy student with a severe intellectual disability problem and he is speech impaired and cannot talk at all. Jake's IEP meeting was performed with the following staff present a Speech/Language Specialist, Physical Specialist, Occupational Specialist, Special Eduacation Trainer and a District Representative. Each group member talked about Jake's goals instruction the Occupational Specialist and Physical Specialist talked about Jake's goals instruction which are based on medical prescription from a doctor. The Language Specialist talked about providing Jake with a speech oriented Tech-8 interaction device. It talks in 8 pre recorded human voices. It also has 8 levels which mean that 8 overlays can be created on different styles or subjects making it possible for Jake to speak 64 commands or messages. The class room teacher talked about Jake's educational goals. Jake's requirements are as follows speech Treatments twice weekly for 30 minutes in a group of 3:1:1 ( to increase peer interaction); Occupational Treatments thrice weekly Individually; Physical Treatments twice per week Independently. The Special Education Instructors and Related Service Providers use the Brigance Analysis which has been recognized in many Special Education Programs as a change assessment for the intellectually disabled.

Short Term Goals

Jake will use a Tech-8 interaction device to ask for items and make requests 20% of time by 2/12, 40% by 4/12, 60% by 6/12, 70% by 8/12, 80% by 10/12 and 90% by 31/12/12.
