Introduction

Practice setting problem

Tom, a 34 year old man, was recently diagnosed with asthma by his doctor. Tom lives with his wife Julie. Tom works in a marketing company. He stays home on weekends, and he and his wife do cleaning and gardening of the house. Recently, Tom suffered with the chest pain and tiredness.

He went to hospital and he came to know about Asthma. Asthma refers to long term disease in which airways are narrowed and in some cases inflamed. Whistling sound during breathing, tightness of chest and coughing are mainly caused by asthma. Upon by questioning by his doctor, he revealed that he has been feeling tired, having trouble sleeping, and frequent breathe changes. Tom attends my clinic and from information I obtained from him, I came to know that he is having frequent respiratory symptoms. His frequent breathe changes refer to the shortness of breath. He also reported that he is having a problem of tightness in the chest. He was suffering with the early stage of asthma. He was having medical treatment of Asthma and having few medications to overcome this disease.

Importance of the clinical problem and its significance to nursing practice

Identification and control of asthma is the first clinical approach, prohibiting the other diseases likewise coughing and wheezing. Drug therapy, is a most beneficial choice in the case of Tom's asthma. Drug therapy is a useful treatment in the case of controlling asthma disease.

Allergy test must be conducted to depict the asthma. Wheezing sounds must be heard as lungs sound are usually the same in the case of asthma patients. Other tests may include the arterial blood gas test, lung function test, chest x-ray and peak flow measurements test. Treatment involves the control of airways hindrance and by keeping yourself away from the triggers. The proper treatment requires the mutual understanding of the patient and doctor. The patient and patient relatives must be taught about the symptoms of asthma and about necessary possible treatment plan. They should work as a team. Long term control drugs as inhalers and beta adrenergic steroids are useful for the severe asthma patients to help them get back to normal life. If a patient suffers a severe asthma attack, doctor should be consulted without delay. Patient may also require hospitalization in the case of severe asthma attack.

Impact on patients and/or a community

Self management is off course mandatory for the asthma patient. Patient and patient relatives are provided with the useful education over the treatment plan of asthma. This treatment plans involves the knowledge of asthma symptoms, and taking a peak flow measurement test, and avoid the triggers of asthma.
