Abstract

This report highlights 'Community definition' and analyses a demographic area within the community. It assesses the health needs of patients living in the community and attempts to answer, “How the primary care meets patient needs?”

The assessment of patients was carried out using a nursing model and other assessment tools. Also highlighted, are the diseases in this area. Rushey Mead has a higher prevalence of Coronary Heart Disease, Diabetes, and Hypertension. These diseases are higher than average in Asian population due to lifestyle and diet.

Community services and multi-agency in this community are explored. Rushey Mead appears to be much deprived although this area has been improving over the past 20 years and more.

Introduction

This report aims to critically analyse the health needs of an individual patient in the community. This report will use nursing model and various assessments to find out the aspect of care regarding the patients' actual problems and needs. The community profile will be explored within demographical and epidemiological link involved in patient care provided. The standard of community services and support network within the area, together with the multi-agency working within the community providing services to patients, will be explored and discussed. The patient nursing assessment will be carried out using a particular nursing model.

Confidentiality

In order to carry out this report, the verbal consent has been acquired from the patient and permission obtained from the mentor as well. This is to adhere with the Nursing and midwifery Councils NMC (2008) code of professional conduct, on maintaining patient's confidentiality. To ensure this, patient will be known as Mrs “S”.

Community definition

A community is considered as a “living” organism with interactive webs of ties among organisation, neighbourhoods, families, and friends. (Clarke, 1999) This definition describe the wider complication involved when inspecting a community.

The community is referred as the people with a basis of common interest and network of personal interaction grouped either based on locality or on a specific shared concern or both, shared common interests each other. (Smithies & Adams, 1990)

A community profile is a complete description of the needs of a population that is defined, or defines itself as a community, and the resources that exist in that community carried out with the active involvement of the community itself, for developing an action plan or other means of improving the quality of life in the community. (Campbell, 2001)

A community profile is a community of health nurses described as “a professional care provider that can tailor individual care for community or community groups. (Wilson, 2004)

Patient Profile

A 63-year-old lady who is a retired garment factory worker and lives alone in Rushey Mead, in a council flat. Betty has two grown up children who live nearby and visit her on regular basis.
