Statement of Problem/Need/Challenge/Concern

The case is concerned about the patient who is 58 years old. He has admitted to the hospital as he is suffering from severe depression caused by the excessive use of alcohol. The patient even tried the number of suicidal attempts to get rid of his life. He reported back pain and rise of LFT's, having hypertension as well. Furthermore he has also caught the Hepetitas C and gastroesophageal reflux. As far as his personal life is concerned then he is divorced with no children. His immediate family is just confined to his two siblings who are settlled in Pennsylvania and Texas. The patient also claims to live in isolation for the past thirty years of his life and as a result he has a long standing history of depression, suicidal ideations, and substance abuse (alcohol). Now he is being treated with number of therapies, examinations and treatments to get his normal life once again. Due to the great struggle of his hospital and his own willingness, he is able to recover and feel betterment. The evaluation of this patient will definitely support him to lead his life perfectly.

The Evaluation of Professional Practice is an organized approach to improving practice of comparing practices regularly performed and the results obtained, with professional recommendations. This approach is inspired by the formative assessment of Anglo-Saxon. The assessment practices resulting in a dual commitment of physicians: first, they undertake to base their recommendations on clinical practice and secondly they measure and analyze their practices by reference thereto. The modalities of implementation of the EPP by doctors or medical teams are many and varied. They are not limited to methods based on measuring a posteriori differences between practice and a repository (audit). They may include approaches that, for example, allow better control the processes of care by making evaluation in daily practice (clinical pathway, reminder or checklist, decision trees, diagnosis or treatment, review of death- morbidity, reviewed records, meeting). They help the patient to fight against his depression, suicidal ideations, and substance abuse (alcohol). Without forgetting the emerging forms of medical practice and evaluated as protocol care networks, the analysis groups or peer practice medical homes, or even devices that could result negotiating contracts as conventional practice or agreements Proper Use of Care (Acbus). EPP training and assessment practices (EPP) and Continuing Education (CE) both have a single purpose of improving the quality of care and service to this patient (Bibb, 2001).

To achieve the same purpose, EPP and FC borrow complementary ways:

•The FC favors a more educational, based on the acquisition of new knowledge / skills

•The EPP favors a more clinical and professional, based on data analysis activity.
