Introduction

It was very difficult to manage patients' records through manual and old ways of keeping records. One major problem was recording and updating records, second, it was very difficult to manage many registers and records with the history of patients and to again reach for that record, whenever required. In order to deal with this system, manual record keeping replaced by computerized system of record keeping. One of the most efficient ways of managing and keeping record is “Summary Care Record” (SCR). This system is based on keeping electronic record, which can be accessed through internet.

It provides breakdown of different products or services provided by the NHS summary case record SCR. It can provide information about four major areas of patient's health care to the health care providers and GP in case of emergency. These four main services that are required in case of emergency are personal information, patient case history, medication, allergies.

Purpose

The purpose of this system is to provide complete information about the patient case history, medication, allergies, and any progress of the health condition (http://www.nhscarerecords.nhs.uk/). These records are very effective in order to provide complete information to the places where a person receives care services. This information is very helpful especially in case of emergencies.

Composition

SCR provides information about the translation and text phone services, it provide complete information about the medication, allergies, and personal details (Greenhalgh et al., 2008). It contains name, address, date of birth, case history, and NHS Number to provide complete details. It also allows options to share other information and discuss issues related to healthcare.

Derivation

Electronic access to the patient information is based on a reflective framework for information system IS, this is to provide quick and easy solution for the complex record system in health care system. It is derived from the diffusion of innovation (Dol) model for complex innovation in healthcare valuable (Dymoke-Bradshaw & Brown, 2010). It is based on the nine components of Greenhalgh et al.'s model (Dymoke-Bradshaw & Brown, 2010).

Greenhalgh et al.(2008) also present consent model that allows people to choose whether they want to have their record in SCR or not. The research shows that many people have no information about SCR.

Format and Presentation

This system provides complete information to the physicians and nurses when they are in emergency condition. It is very difficult in manual and in other systems to access the data quickly. General physician, nurses, health care staff, and only concerned authorities have access to this data. This system also ensures safety and privacy by providing special NHS number and NHS Smartcard with PIN to access information. It can also assist a person to consult his or her GP, even when not in England. The information is shared through letter, email, fax or ...
