Introduction

The Australian Primary Care Collaboratives Program is funded by the Commonwealth Department of Health and Ageing and provides service through the Improvement Foundation Australia. The main purpose of this program is to promote and encourage general medical practices in Australia by delivering quick, measurable, sustainable and systematic development and i9mprovement in the healthcare that they provide to their patients by understanding and using effective implication of quality improvement methods and skills.

The main objective of this program is to maintain the quality of healthcare by bring improvement in clinical practices and outcomes and provide good health to those patients who are suffering from chronic and complicated conditions. This is done by promoting and encouraging quality improvement in primary healthcare sector. It has been observed that more than 600 practices have taken part in the Collaborative from the year 2005 and additional 1000 have been added as the opportunity during the years 2008-2009. The results revealed that there has been an improvement in patient outcome and system of healthcare practices particularly with patients of diabetes, coronary heart disease and the availability of doctors for effective care of patients on time (http://www.wml.org.au, 2012, pp. a1-2).

Goal: the main purpose and goal of the Australian Primary Care Collaboratives Program is to improve the health services by providing access to diabetes and coronary heart patients.

Plan, organize and apply the assessment process and assessment tools for each network and primary healthcare services, respecting the meaning of these in the context of continuous quality improvement.

The criteria developed for each device, are the result of:
