Issues: Patients Re-admittance

The statistics show that at least 20% of the patients admitted in the hospitals are re-admitted within 30 days (Krames, 2012, p.1). There has been a significant rise in understanding the need for readmitting a patient into the hospital. The re-admittance and the treatment costs are increasingly burdening the taxpayers due to negligence on behalf of the hospitals during the treatment and post treatment care (Campbell, 2012). The major reasons identified for the readmission of the patients are demographics of the patient, which includes the sex of patient and, living conditions and unavoidable relapse. These issues lie at the end of patient. The responsibility of the hospital is to ensure that patient is responsible and follows the direction for the post discharge treatment. Premature discharge and poor patient compliance are the reasons, which lie at the end of hospital (Morrison & Monagle, 2009, p. 235).

The readmission issues are avoidable if necessary measures are adopted from the hospital. The measures include improved support from the community and follow up for the patient condition after discharging the patient and providing them with sufficient knowledge to recover and avoid the symptoms. Ensuring that nurses regularly visit the patients and reports, the improvement or deterioration observed (Hasan, 2001, p.177).

Many hospitals are being fined in for readmitting the patients within 30 days of discharges. The most common health conditions for re-admittance include the heart failure, pneumonia, psychosis and chronic obstructive pulmonary disease.
