Case Review

Introduction

The purpose of writing this paper is to readers a highlight regarding the medical case of a patient suffering from different sort of spinal cord pains, therefore, the author recommended her to go through number of different tests for different spinal cord problems. To physicians reviewed the patient thoroughly and came up with different tests and presented the evidence on the bases of those procedure. In addition to that, the author also presented the background of the patients so the readers could easily understand the condition of the patient.

Patients Background

My patient is 34 yrs old female called 'Melanie Jones', who attended a GP practice where I worked as a First clinical contact / Nurse Practitioner in 'Sale, Grt Manchester', from April 2012 to Oct 2012. She attended with her 8 yr old daughter who Ms Jones had rang that morning requesting an on the day slot for her child, who had cough /cold symptoms, I agreed to see her child at 12.00 that day to review and assess her. The patient alert system was basic and involved the clinician [me] going into the waiting room and calling the patients name, whilst doing this I noticed it took the child's mum a long time to get up out off the chair and struggled to walk the short distance to the consultation room, she needed the wall to support her along the corridor. I asked her if she was OK' she replied ' she'd hurt her back', I assessed her daughter who was diagnosed with a Viral URTI given advice and a time frame+red flags for review.

Mum appeared to struggle during her daughter's consultation with pain/discomfort, so I asked some more questions regarding her condition as she was a patient of the practice. Main points:

Over the counter meds not managing pain

urinary incontinence and faecal soiling

paraesthesia to feet/saddle

Limp

No previous med /surg hx, meds or allergies.

No trauma

Based on the evidence, the patient gone under number of different treatments which the author discussed below.

Clinical Reasoning

Lumbar spinal stenosis

Lumbar spinal stenosis is defined as a relative or absolute narrowing of the nerve root canal. This narrowing causes symptoms typical in the sense of neurogenic claudication, called claudication. This leads to an individual patient for the route to a pain in the legs that makes a move on impossible. In the Melanie Jones case, the disease in the lumbar spine, it occurs but also in the thoracic and cervical spine. The symptoms are varied and depend on the location and extent of the narrowing. Due to this, it includes pain, discomfort, but also paralysis. Relieved by bending forward, sitting, lying down, biking, and the symptoms again, as in this case extended to the spinal canal slightly. When bending backwards of the spine, it is more likely to be an additional narrowing of the canal, so that increase the pain.
