Presenting Problems and Strengths

Jonathan Reid, the patient in the case is 19 years of age and was provided with residential attention and treatment at Tucson, Arizona on December 07, 2012. Jonathans regards himself as a multi-racial, heterosexual, Caucasian male. He is of the view that he is a creative individual who has been able to maintain a cordial relationship with his family and friends. In his family are his father and two sisters. Jonathans has revealed that he is at ease at the facility and willingly cooperates with the staff at the center. However, he holds little hope of ever being able to maintain the behaviors he has learned at the center at that have helped him in coping with acute depression and chronic disorders.

Upon review of Jonathan's past record in the Individualized Education Plan, as of August 21, 2011, it was seen that Jonathan has shown an array of symptoms leading to disturbed behaviors (www.mayoclinic.com, 2012). Upon further evaluation, it was found that Jonathan manifested symptoms of acute depression and had also attempted to end his life on two separate occasions in the previous month. According to the discussion with his father and colleagues on January 03, 2012, it was revealed that Jonathan was highly susceptible to injuring himself to the point that it may end his life. It was due to this tendency that Jonathan was admitted for treatment.

Brief History

The patient named as Jonathan Reid has been living with his family prior to December 2010. His family members included his mother, father, and two younger sisters and they have been residing at San Antonio, TX. The patient was observed to be performing very poorly in terms of academic achievements and later it was found out that he was suffering from a learning disability since 3rd grade. Therefore, he began to receive educational services accordingly and gradually picked up the speed and improved in gaining education with the help of IEP until sophomore year in high school. As per his records of sophomore year in October 2010, where the sad incident happened and it was recorded that he was driver of the car in which his mother died and his sister got serious physical injuries. Thereafter, the school record show that Jonathan has been performing very badly in studies and began to display alternating non-attendance from class. As per records of metal health services, it was observed that he began to take marijuana two to three times in a week and was thereby displaying symptoms of depression. The reliability of these reports was confirmed from Jonathan himself.
